Here's something I've never figured out.

Most of the time when there is an offensive holding it is 10yds and repeat down

Sometimes (when the hold is down the field), it's a 10yd penalty, but they count the down and yardage gained.

I just saw one in the Dolphins game, where I really thought it should have been the first kind, but they called the second kind. So what exactly is the rule?
