A lot of people were openly questioning Biden's capacity, it was kind of a big deal and he had to withdraw from the campaign because of it.
Sometimes stylized eagles are just stylized eagles and not Nazi eagles. Sometimes lightning bolts are just lightning bolts and not SS signs. Sometimes, crosshair symbols are just crosshair symbols and not Celtic crosses. Sometimes, awkward, straight arm, palm-down gestures are just awkward gestures and not Roman salutes.
But when you start to view people who are sensitive about such imagery as political villains, and when you start to view trolling your political villains as some sort of art form, it's amazing how easy it is for this sort of thing to become part of your repertoire.
Was Elon expressing actual support for National Socialism? Definitely not. Does Elon think that ambiguous Nazi references are something that people should take great care to avoid? Probably not. Is Elon capable of doing wildly offensive things in an effort to troll his detractors? Definitely. Was this gesture done deliberately for that purpose? Who knows.

The original Nazi salute as used by the actual Nazis didn't start with a hand over the heart before raising the arm, so it's not a literal carbon copy at all.
